Our Wilson community health center is located in the heart of the Uptown neighborhood. Through an agreement with the Chicago Department of Public Health (CDPH) we are able to meet the community’s need for more accessible and affordable health services. At Wilson, we provide primary care and mental health services to patients whom are uninsured, underinsured, immigrants, and refugees.

Sonrisa Family Dental provides dental services at our 845 West Wilson Avenue location.

LOCATION HOURS:

Day	Time
Monday – Friday	8:30 am – 5:00 pm
Saturday	8:30 am – 12:30 pm (1st and 3rd Saturday)
