Our Insurance Policies

You can be a patient with Medicaid, a Marketplace, employer or other insurance plan, or no insurance. As a federally qualified health center, we guarantee access to care regardless of ability to pay.

Uninsured? Pay Based on Your Income

Tapestry 360 Health offers sliding-scale fees to anyone who does not have health insurance. You pay a discounted fee based on your household size and income.

To receive the sliding-scale fee, please bring with you to your visit proof of income for the past 30 days. If you are self-employed (taxi driver, etc.), bring your most recent tax return. If you have no income, a notarized letter from the person supporting you will be required.

The sliding scale fee starts at $30 and includes a visit with the medical provider, x-rays and labs, if ordered. If you are prescribed medication, our staff will work with you to obtain medicine at an affordable cost.

We're making a few important updates to ensure that our sliding fee scale program continues to support as many patients as possible.

What’s Changing?

Starting April 17thall patients—whether insured or uninsured—must complete a Sliding Fee Scale application and provide proof of income.

This ensures we can determine if you qualify for a discount and help keep your healthcare affordable, now and in the future.

  • Blue Cross Community Family Health Plan
  • Community Care Alliance (Dual eligible only-Medicaid/Medicare)
  • County Care
  • Humana (Dual eligible only -Medicaid/Medicare)
  • Meridian Health Plan
  • Molina

Eligible individuals may apply for coverage through Medicaid any time. Medicaid programs include All Kids, Moms & Babies, Adult ACA, and AABD. Our Benefits Navigation Specialists can help you apply for these programs. Eligibility criteria vary depending on the program:

Illinois Family Planning Program

The Illinois Family Planning Program (IFPP) is a State funded program through the Illinois Department of Public Health (IDPH). The program provides sexual and reproductive health care and contraceptive supplies for free or at a low cost to all patients who want and need them! No insurance necessary.
