Our Lincoln Square community health center opened its doors in the spring of 2009. We provide a medical home to some 4,000 patients in the Lincoln Square, Albany Park and Irving Park community areas.

LOCATION HOURS:

Day	Time
Monday - Wednesday, Friday	8:30 am – 5:00 pm
Thursday	8:30 am – 8:00 pm
Saturday	8:30 am – 12:30 pm (1st and 3rd Saturday)
