The community around us plays a big role in the state of our health. We can help you find local social-service agencies and groups
Today we know that the Zip code where you live is one of the main predictors of your life expectancy. Access to decent affordable housing and neighborhood safety are just a couple examples of how where we live affects our health.
As your primary care providers, we aim to support your whole health. We may not provide services you need – such as help with food or housing, or if you are experiencing a mental health or other crisis. But we may be able to refer you to another agency that can help.
For example, our Behavioral Health Consultants can suggest options for seniors, such as caregiver programs. We can also recommend options if you are looking for psychotherapy or other long-term counseling. Below are some organizations we work with regularly:
