At Tapestry 360 Health, we believe that everyone deserves access to high-quality healthcare—regardless of income or insurance status. That’s why we offer a Sliding Fee Scale Program that adjusts the cost of care based on your income and household size.
To make sure this program continues to support as many patients as possible, we’re making a few important updates.
What’s Changing?
Starting April 17th, all patients—whether insured or uninsured—must complete a Sliding Fee Scale application and provide proof of income.
This ensures we can determine if you qualify for a discount and help keep your healthcare affordable, now and in the future.
How Do I Apply?
It’s easy! You can pick up an application at the front desk during your next visit. Be sure to bring proof of income so we can complete your application quickly.

What if I don’t complete the application?
If you’re uninsured and choose not to submit the application and proof of income, you’ll need to pay the full cost of your visit upfront.
What if I forget my proof of income?
Don’t worry—we’ll still see you. You’ll be charged the full fee, but if you submit proof within 10 days, we’ll review and adjust your balance based on your eligibility.
I have insurance. Why do I still need to complete this?
Even insured patients may qualify for additional savings. If you lose coverage or face a high deductible, having a completed application on file can help reduce costs when you need it most.
What counts as proof of income?
Bring one of the following:
- Most recent paycheck stubs (1 month)
- Current tax return
- Award letters or statements for:
- Social Security or Disability
- Unemployment/Workers’ Comp
- Retirement or pensions
- Child support or alimony
- A notarized letter verifying your financial status or housing situation
- For self-employed individuals: current tax forms and a profit & loss statement
- For day laborers: a 1-month daily log and available receipts
