As part of a University of Illinois at Chicago (UIC) study, Tapestry 360 Health and Centro Romero are inviting adults with type 2 diabetes to participate in a virtual interview. The goal of the study is to better understand how diabetes care can be made more convenient, accessible, and supportive for patients like you. Your experiences and perspectives can help to improve diabetes care and make it easier for patients to get the support they need. 

What participation involves 

• A 1-hour virtual interview with UIC researchers 

• A chance to share your experiences with diabetes care, self-management, and support 

• An opportunity to help researchers better understand what makes care easier (or harder) for patients 

Who can participate? 

You may be eligible if you are: 

• 18 years or older 

• Living with type 2 diabetes 

• Receiving care at a Federally Qualified Health Center (FQHC) in Illinois, such as Tapestry 360 Health

If you are eligible, consider joining a 1-hour virtual interview and sharing your perspective. Interested participants can sign up here. People who complete the interview will receive a $40 gift card for their time.
