Check Your Blood Pressure at Home
Tapestry 360 Health offers free blood pressure machines to our patients who have high blood pressure and qualify. If you’re interested, ask your Tapestry 360 Health team during your visit.
The A&D blood pressure machine box includes Heart Track App for phone, a manual and quick start guides in English and Spanish. You can share your BP results from your Heart Track app with your clinician during your clinic visit without having to write your numbers on a paper log.
The best way to check your blood pressure
- When you check your blood pressure, check it twice and wait one minute between checks
- Check your blood pressure in the morning and evening for 3 days in a row
If you don’t have a smartphone or can’t connect your A&D BP machine to your phone app, you can record your BP numbers on paper. You can download and print this paper BP Log Bring your paper log and share it with your Tapestry 360 Health Provider at your next clinic Telehealth visit.
