$240 million in funding will launch and expand behavioral health care services

Integrated care is more than a buzzword; it’s a lifeline for patients experiencing complex health challenges. Thanks to a groundbreaking grant from the Biden-Harris Administration, our organization is grateful to expand and enhance our services, providing holistic care that addresses mental health, substance use disorders, and primary care as interconnected facets of well-being.

This funding couldn’t come at a more critical time. The need for accessible, high-quality behavioral health services has never been greater. Many of our patients face barriers such as stigma, lack of insurance, or cultural obstacles that prevent them from seeking care. By integrating mental health and substance use disorder treatments into primary care, we ensure that every door becomes the right door for accessing comprehensive health services.

Advancing Health Equity Through Action

At our health centers, integrated care is more than a model; it’s a commitment to serving the whole person. With the support of this grant, we’ve already made significant strides:

  • Expanded Behavioral Health Team: We hired a behavioral health consultant to increase access to services, alongside two behavioral health care coordinators who collaborate with the behavioral health team to refer patients to higher levels of care and address health-related social needs. These coordinators ensure patients receive the care and services they need by closing the loop on referrals.
  • Community Education: We’ve begun educating community residents about our integrated primary and behavioral health care model and offering group classes on behavioral health topics to foster understanding and reduce stigma.
  • Staff Training: Newly hired behavioral health consultants and care coordinators have participated in targeted training sessions focused on behavioral health and substance use disorder, ensuring they are fully equipped to support our patients.
  • Community Partnerships: Through a partnership with Street Samaritans, an organization dedicated to street outreach for homeless individuals, we’ve streamlined efforts to connect individuals with substance use disorder who are ready for treatment into care at Tapestry 360 Health.
  • Peer Support Programs: We’ve introduced peer support services for patients with substance use disorder, offering them a compassionate and informed guide through their recovery journey.
