What are my rights as a patient?
At Tapestry 360 Health, your rights are at the center of all that we do. You have a right to:
- Polite, respectful, and quality care
- Know about your treatment plan
- Know your health care team
- Make decisions about your care
- An interpreter
- Privacy
- Comfort and safety
What are my responsibilities as a patient?
To help us make sure we give you the highest quality medical care, we ask that you:
- Participating in self-management goal activities
- Please provide us with all the information you can about any health issues you are experiencing, including your medical and vaccination history, allergies, medications, home remedies, and any other relevant details that may assist us in your healthcare.
- Let your provider know how well you understand what they are saying to you. Let them know if you do not understand any part of what they are saying.
- Do your best to follow the plan of care that your provider develops with you.
- Please provide us with your most up-to-date contact information.
