Here, you’ll find information about your rights as a patient—what you can expect from us—as well as your responsibilities as a partner in your care. We’re committed to creating a respectful, supportive environment for everyone.

What are my rights as a patient?

At Tapestry 360 Health, your rights are at the center of all that we do. You have a right to:

  • Polite, respectful, and quality care
  • Know about your treatment plan
  • Know your health care team
  • Make decisions about your care
  • An interpreter
  • Privacy
  • Comfort and safety

What are my responsibilities as a patient?

To help us make sure we give you the highest quality medical care, we ask that you:

  • Participating in self-management goal activities
  • Please provide us with all the information you can about any health issues you are experiencing, including your medical and vaccination history, allergies, medications, home remedies, and any other relevant details that may assist us in your healthcare.
  • Let your provider know how well you understand what they are saying to you. Let them know if you do not understand any part of what they are saying.
  • Do your best to follow the plan of care that your provider develops with you.
  • Please provide us with your most up-to-date contact information.

Tapestry 360 Health receives an HHS Health Center Program grant as a grantee under 42 USC 254b. It has Federal Public Health Service (PHS) deemed status concerning certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

In a Primary Care Medical Home (PCMH), we provide primary health care that’s centered around you, ensuring accessible, coordinated, high-quality care based on your identified health needs and goals.

Each patient will work closely with a dedicated group of staff (including but not limited to providers, nurses, medical assistants, front desk, and behavioral health consultants) whose primary goal is to achieve coordinated, high-quality care.

You are the most important member of your care team because no one knows your body better than you. We want you to understand, agree with, and participate in your treatment plan. To ensure that this happens, your provider will:

  • We encourage you to set goals and work with us to achieve them.
  • Provide you with a visit summary which includes your medications, diagnoses, and plan of care.
  • Communicate when you should schedule your next appointment.

You can be referred to services outside of Tapestry 360 Health by working together with your healthcare provider to determine the best plan of care. If you are referred outside of our organization, we will work with you and your outside care providers to ensure all of your medical records are in one place.

The patient portal is a secure online website that gives you convenient 24-hour access to your personal health information and medical records—called an Electronic Health Record or EHR—from anywhere with an internet connection.

All information related to your care is confidential and will be kept private. Your medical records will not be shared without your consent unless required by the law. You will be given a copy of our privacy practices during your orientation, and you can request another copy at any of our health centers at any time.

First, meet with your primary care provider to establish care at Tapestry 360 Health. At that time, let your provider know you want to see the behavioral health consultant, or psychiatrist. He or she will place a referral in your chart, and you will be able to schedule with our behavioral health team.

Many Tapestry 360 Health staff members are bilingual in Spanish and other languages. If we do not speak your language, we have telephone and video interpretation in over 200 languages, available to you at no charge.

Tapestry 360 Health also offers sliding-scale fees to anyone who does not have health insurance. You pay a discounted fee based on your household size and income.
