En Tapestry 360 Health, creemos que todas las personas merecen acceso a atenci\u00f3n m\u00e9dica de alta calidad, sin importar su nivel de ingresos o estado de seguro. Por eso ofrecemos un Programa de Escala de Tarifas Reducidas<\/strong><\/a>, que ajusta el costo de la atenci\u00f3n m\u00e9dica seg\u00fan sus ingresos y el tama\u00f1o de su hogar.<\/p>\n

Para seguir apoyando a la mayor cantidad de pacientes posible, haremos algunos cambios importantes.<\/p>\n

\u00bfQu\u00e9 est\u00e1 cambiando?<\/h3>\n

A partir del 17 de abril<\/strong>, todos los pacientes\u2014con o sin seguro\u2014deber\u00e1n completar una solicitud de Escala de Tarifas Reducidas y proporcionar comprobante de ingresos.<\/em><\/p>\n

Esto nos permite determinar si califica para un descuento y ayuda a mantener su atenci\u00f3n m\u00e9dica asequible, ahora y en el futuro.<\/p>\n

\u00bfC\u00f3mo solicito?<\/h3>\n

\u00a1Es f\u00e1cil! Puede recoger la solicitud en la recepci\u00f3n durante su pr\u00f3xima visita. Aseg\u00farese de traer su comprobante de ingresos<\/strong> para que podamos procesar su solicitud r\u00e1pidamente.<\/p>\n

\u00bfQu\u00e9 pasa si no completo la solicitud?<\/strong>
Si no tiene seguro y decide no entregar la solicitud ni el comprobante de ingresos, deber\u00e1 pagar el costo total de su visita por adelantado.<\/p>\n

\u00bfQu\u00e9 pasa si olvido traer mi comprobante de ingresos?<\/strong>
No se preocupe\u2014igual lo atenderemos. Se le cobrar\u00e1 la tarifa completa, pero si entrega el comprobante dentro de los 10 d\u00edas, revisaremos su caso y ajustaremos su saldo si corresponde.<\/p>\n

Tengo seguro. \u00bfPor qu\u00e9 tambi\u00e9n debo completar esto?<\/strong>
Incluso con seguro, usted puede calificar para descuentos adicionales. Si pierde su cobertura, enfrenta un per\u00edodo sin seguro o tiene un deducible alto, tener su solicitud completada puede ayudarle a reducir los costos.<\/p>\n

    • Carta notariada que verifique su situaci\u00f3n financiera o de vivienda<\/li>\n
    • Para trabajadores por cuenta propia: declaraci\u00f3n de impuestos del a\u00f1o en curso y un estado de p\u00e9rdidas y ganancias<\/li>\n
    • Para jornaleros: registro diario de 1 mes y recibos (si tiene)<\/li>\n<\/ul>\n
